How do I access my home exercise program?

Following your first evaluation, you will be sent a link via provided email to access your care plan created by your physical therapist. Please check both inbox and spam, as this will come from the sender strivehub.com

I created an account years prior and began physical therapy again. What should I do if I cannot remember my log in information to strivehub.com?

Confirm with your physical therapist the email linked with your account and follow prompts to re-set your password.

I want to change the email associated with my account, who can help me with this?

Please contact strivehub.com directly to adjust your account settings.

My child is attending physical therapy, although they are < 18 years old, can the home exercise program be under their email?

Yes, with parental consent the home exercise program can be linked to a minor’s email. If you have started care already, and you decide to modify the email on the account from parent to child, in order for the child to be able to successfully log in, you will need to contact the strivehub team directly for account email update.

My treatment was progressed by my physical therapist during my visit and I don’t see new instructions in my exercise portal. What should I do?

Contact your physical therapist via email to check in and verify your home exercises are up to date. Alternatively, call our front desk reception team and they will communicate directly with the physical therapist.

My home exercise program is too challenging, timely, and/or causing me pain. Should I stop doing it?

Communicate with your physical therapist at your next scheduled session or via email about your home exercises in order for them to modify your program and help tailor to your specific needs.
