WELCOME TO QUINN ORTHOPEDIC PHYSICAL THERAPY
We are pleased to be your chosen provider for rehabilitation services and anticipate building a positive relationship with you. You will find our therapists skilled and professional, and our support staff friendly and efficient.
Patient registration begins with demographic verification, including insurance coverage. Our team will collect your policy details as such our billing department is able to provide an informative summary of your benefits.
Please complete our New Patient Intake Information Form. As an alternative to an online submission, you are always welcome to contact our team by phone.
To finalize registration and prepare you for treatment, your complete packet will be sent to the email address provided. If you do not receive your intake packet via email 24 hours before your scheduled appointment, or you notice a clerical error, please contact our office.
Necessary to begin treatment, we encourage you to sign your intake documents within 24 hours from receipt. Failure to complete may require us to reschedule your appointment.
As we have now gone paperless, we would prefer that all supplemental documentation for your care (insurance card, ID, and valid PT referrals) is sent by email to reception@quinnpt.com or faxed to 408-252-1159.
